Indication Basics

Tadalafil is a medication used to treat erectile dysfunction (ED) in men. It works by increasing blood flow to the penis, helping to achieve and maintain an erection when sexually aroused. It belongs to a group of drugs known as phosphodiesterase type 5 (PDE5) inhibitors.

Tadalafil belongs to a group of medications called phosphodiesterase type 5 (PDE5) inhibitors. These drugs are classified as PDE5 inhibitors based on their mechanism of action, which is to block the action of an enzyme that normally breaks down cGMP, which is necessary for the breakdown of blood vessel-penetrating tissues.

Tadalafil is available in tablet form and is taken orally. It is important to follow your doctor’s instructions about dosage and duration of treatment. Tadalafil may take several weeks to start working. If you miss a dose, take it as soon as you remember. However, if it’s almost time for the next dose, skip the missed dose and continue with your regular schedule. Do not take a double dose to make up for a missed one.

Interactions

A. Top Drug-Drug Interactions:

1.Nitrates (e.g., nitroglycerin, isosorbide dinitrate): Cialis and nitrates both lower blood pressure. Taking them together can cause a significant and potentially life-threatening drop in blood pressure (hypotension), leading to dizziness, fainting, or even heart attack or stroke. It is contraindicated to use Cialis with nitrates or recreational "poppers" (amyl nitrites).

2.Alpha-blockers (e.g., prazosin, terazosin, tamsulosin): Both Cialis and alpha-blockers relax blood vessels, which can lead to excessive blood pressure reduction when used together. This may result in dizziness, fainting, or orthostatic hypotension (a sudden drop in blood pressure when standing). If needed, your doctor may adjust the dosage or recommend spacing the medications.

3.Other antihypertensive drugs (e.g., atenolol, propranolol, enalapril, lisinopril): Cialis can enhance the blood-pressure-lowering effects of these medications. While this interaction is usually less severe than with nitrates or alpha-blockers, it can still cause dizziness or fainting in sensitive individuals. Close monitoring of blood pressure is recommended.

Other Drug-Drug interactions:

  • Ketoconazole, itraconazole (used to manage fungal infection)
  • Protease inhibitors (used to manage HIV infection) Ex. ritonavir, lopinavir
  • Phenobarbital, phenytoin, carbamazepine (medicines used for fits)
  • Rifampicin, erythromycin, clarithromycin (medicines used for bacterial infections)
  • Other medicines used to improve blood flow to the penis (Ex. sildenafil)

B. Drug-Food Interactions:

Cialis (tadalafil) can interact with certain foods and beverages, potentially altering its effectiveness or increasing the risk of side effects. Here are the key drug-food interactions to be aware of:

  1. Grapefruit and Grapefruit Juice: Grapefruit can increase the levels of tadalafil in your blood by interfering with its metabolism. This may amplify side effects, such as headaches, dizziness, or low blood pressure. Avoid consuming grapefruit or grapefruit juice while taking Cialis.
  2. Alcohol: Drinking alcohol with Cialis can enhance the blood pressure-lowering effects of the medication, leading to symptoms like dizziness, lightheadedness, or fainting. Limiting alcohol intake is recommended while using Cialis.
  3. High-Fat Meals: While Cialis can be taken with or without food, consuming it with a high-fat meal may delay its onset of action. Opt for lighter meals if rapid effectiveness is needed.
  4. Caffeine: Though not a direct interaction, combining Cialis with high caffeine intake might worsen side effects like headaches or increase heart rate in sensitive individuals.

A new study shows that men can use Cialis to treat erectile dysfunction.

The study, published online in the British Journal of Urology, analyzed data from more than 25,000 people who used the drug for more than a year.

A total of 969 participants were diagnosed with erectile dysfunction and were given the drug Cialis. They then compared the results to an analysis of data from a larger study called the International Index of Erectile Function.

In the study, people who used the drug were more likely to use it to have erectile dysfunction, as they were more likely to have had erectile dysfunction when they were in the first year of treatment.

The study also found that erectile function was worse in those who had used the drug, compared to those who didn't.

“The results of the study show that the effects of Cialis can be long-lasting and can be used successfully in the treatment of erectile dysfunction,” said study co-author Dr. Steven Nissen, director of the University of Toronto’s Centre for Sexual Medicine. “In clinical trials, Cialis is effective in treating the symptoms of erectile dysfunction.”

The researchers found that men who took the drug had about three times higher rates of ED.

Cialis can be taken with or without food, though the drug is more commonly used in people with diabetes.

“The study also found that Cialis helped to reduce the risk of cardiovascular events, such as heart attacks and strokes,” said Nissen.

“This can be especially helpful for patients who have high blood pressure,” he added.

A new study has also found that people taking Cialis, or sildenafil, to treat erectile dysfunction can take it to help the blood vessels in their penis to relax. They can then use it to increase blood flow.

“We found that patients who took Cialis, or sildenafil, to treat erectile dysfunction had an increased risk of erectile dysfunction when they were first started on the drug,” said Nissen.

“This is very important because the drug can be used to treat pulmonary arterial hypertension, which is high blood pressure in the arteries of the lungs. If a patient has a high blood pressure that is not treated with Cialis or sildenafil, this could cause the patient to be unable to get or keep an erection.”

The findings were published online March 26.

According to Dr. Daniel Berenson, a senior professor of urology at Toronto’s Dalhousie University, the drug can help men have an easier time getting and maintaining an erection.

“It’s very important for men to get and keep an erection. This is what the men who use the drug are most likely to do,” Berenson said.

Berenson added that the study is important because it showed that men who took the drug were about twice as likely to use it to have erectile dysfunction.

“But even more importantly, it showed that the effects of Cialis can be long-lasting and can be used successfully,” he said.

“The results of the study show that the effects of Cialis can be long-lasting and can be used successfully.”

The researchers also looked at data from the first and second year of treatment of men with ED.

The study, published in the journalJAMA Internal Medicine, looked at data from about a million users of the drug, from those who took it and also from those who didn’t.

The researchers found that men who took the drug were about twice as likely to use it to have ED.

“These are people who are using Cialis, and they’re using this medication to treat erectile dysfunction,” said Nissen.

“This study shows that the effects of Cialis can be long-lasting and can be used successfully.”

Nissen said it’s important for men to talk to their doctors about taking the drug and how they can improve their sexual health and ED.

“There is a lot of misinformation out there, but that’s really what we’re doing here,” he said.
